Active Measures for First Nations

On behalf of the team here at Abenaki, I am very excited to tell you about a training opportunity to help you with the implementation of Active Measures within your community.

Active Measures has been an integral part of our First Nations Management Suite of Products since 2003 and the introduction of our Case Management module. Our Case Management module provides you with an easy-to-use tool that enables you to record the intake and assessment details for each of your clients. This can include items such as:

  • Personal information

  • Information from prior assessments

  • Action plans and the client’s progress with the plan(s)

  • Employment background, education and training history and skills.

  • Any barriers that a client may have to achieving success

  • Services and resources to which the client has been referred

Case Management can also help you perform the following tasks:

  • Design an action plan for your clients which addresses their goals and objectives. This action plan can be shared on a secure basis with members of a client’s support team (Education & Training staff; Health department; Income Assistance, etc.) so that processes are not duplicated and your members have the benefit of coordinated case management.

  • Establish milestones and report on progress toward achieving the client’s goals so that you can work with them to overcome any obstacles

  • Evaluate your First Nation’s progress insofar as the quality and quantity of program activities that have been provided and the outcomes of these activities.

  • Generate reports to help monitor your case load and results of any Active Measures in place.
